Fail to design = design to fail

At last week’s Entrepreneurship 101 lecture, Steve Carkner of Panacis Medical presented an introduction to the design process: a comprehensive, organized approach to linking customer needs to product design and development.

There are many horror stories out there about failure in the product design process (my favorite is the original Hubble telescope, a billion-dollar piece of space optics which, discovered only after launch, had been built with the wrong focal length). Is there anyone out there who can share similar design faux pas? (Anonymous submissions accepted!)
